I don't know but I remember the incident.  Think there were deaths and other things that happened.  I'm not gonna cheat and won't google it till later.

It is Vic Morrow he and two child actors were decapitated on the early hours of July 23 1982. While he was trying to carry the kids as his character to safety in the Vietnam War thing. As his last words reportedly were "I should have had a stunt man do this."

He is the father of Jennifer Jason Leigh BTW from the Hateful 8. As both were estranged at the time of his death. She changed her name not to be labeled as his kid.

There were lawsuits as well as that of a criminal trial with the director and three or four others. As they were found not guilty of manslaughter. As the lawsuits were settled out of court. 

But it also had a wake up call as the thing called Computer Graphics Imagery or C.G.I. was introduced due to this.
